Welcome to Hounsfield Surgery Online

Hounsfield Surgery serves the whole of Sutton-on-Trent as well as its surrounding villages. Our team includes two GPs, one Nurse, one Health Care Support Nurse as well as our practice manager, reception, administration and dispensing staff.

We offer a full general practice service and run specialist clinics for children and pregnant women, diabetes and asthma sufferers and for patients needing minor surgery. We also offer an on-site dispensing service.

At Hounsfield Surgery, we aim to treat all our patients promptly, courteously and in complete confidence.

This website is for both existing patients and those considering registering with us. It tells you about our services, how to access them and some general information about how our practice operates.

If you live in our practice area and would like to register with us, please complete one of our registration forms and health questionnaire, which are both available from our reception.

Try the new NHS App

If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.

You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.

If you already use SystmOnline you can continue to use it. You can use the NHS App as well.

Did you know you can book an Extended GP Access appointment?

Lombard Medical Centre and Collingham Medical Centre are working in partnership to offer additional evening GP appointments as well as Saturday morning appointments.  These appointments are available to Newark residents who are registered with a GP Practice in Newark.

Evening appointments are available on Mondays at Collingham and Tuesday to Friday at Lombard Medical Centre from 6.30pm-8.00pm.

Saturday morning appointments are available on a rota basis.

Please note that these appointments are for non-emergencies and are pre-bookable only.

Did you know you can now book GP Appointments in Mid Nottinghamshire on evenings and weekends?

All GP Surgeries across Mansfield, Ashfield, Newark and Sherwood can now offer evening and weekend routine GP appointments.  These appointments are available to all residents who are registered with a GP Practice in Mid Nottinghamshire.

The appointment may not take place at your usual GP surgery, but at another nearby. Your GP surgery will be able to provide you with more information about the location at the time of booking an appointment. 

Appointments are available up to 8pm on a weekday, Saturdays mornings and Sundays from 9am – 1pm.    

Please note Sunday appointments are currently only available at Forest Medical Practice, Rosemary Street Health Centre, NG19 6AB. This is located just off Chesterfield Road very close to the Civic Centre with free car parking available. There is a pharmacy next to the surgery, which is open until 10pm 7 days per week. The surgery is accredited as Dementia friendly, has disability access and is within easy reach of the town centre.  We are currently working with partners to look at possibilities in Newark and further information will be available on our websites shortly.

If you find that you no longer need your appointment please cancel it so that these can be offered to other patients.

Policies on access to records

You can find our policy regarding Patient access to their GP Record, Summary Care Record and GP2GP Record Transfer here. You are now able to request access to view your detailed coded record online via SystemOnline. If you are already registered to order prescriptions and book appointments you can request this access once logged in or if you require access, please enquire in the surgery, bringing photo ID unless practice staff know you well. This access will allow you to see entries that have been entered onto your record using codes but will not at present allow you view the free text narrative. You can also see the results of some tests online. 

Named Allocated GP

Each patient is allocated to a named GP within the practice. This doctor will show on certain documents generated within the practice such as prescriptions. This GP has nominal responsibility for overseeing your care. You can however book to see any GP within the practice. Should you wish to confirm your named GP or change your named GP please contact us.

Friends and Family Test

The Quality of the service that the practice provides is now monitored by using a simple survey called the Friends and Family Test which we would appreciate it if you could complete here.

Online Booking for Registered Patients  

To use SystmOnline you will need to come into the Surgery in person and request a Username and Password.

SystmOnline will allow you to;

  1. View future and past appointment that you have with the surgery
  2. Request your repeat medication
  3. Book and cancel appointments
  4. Change your contact details - When changing your address you will need to provide us with the necessary documents before any changes can made to your record (A utility bill with your name and address showing that is dated within the last 3 months)
  5. Questionnaires - any questionnaires that the practice is using will be available here
  6. From March 2016 we will also be rolling out more detailed access to records for patients who desire this in keeping with NHS England's Patient Online initiative. Pilot schemes suggest that this is desired by only a minority of patients so if you wish it enabled please speak to your doctor. 

 

We now have a GP appointments and telephone advice appointments available to book online.  If you are not already registered for SystmOnline for your repeat medication, please come to reception for a username and password. 

Once you are registered click on the "SystmOnline" logo above to go to the website and follow the instructions from there.
