Dear Patient,

patient surveyWe would like to know how we can improve our service to you and how you perceive our surgery and staff.

To help us with this, we have a Patient Participation Group and a Virtual Patient Representation Group so that you can have your say. The Group meet quarterly after surgery and the Patient Participation Group will ask the members of the Virtual Representation Group some questions from time to time.

We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.

If you would like to join the PPG Group please give your email address to the Surgery so that we can pass it on to the PPG Group who will then send out the invites to the meetings, agenda and minutes.

If you would like to become a virtual member and are happy for the PPG to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.
